WAIVER OF LIABILITY RELATING TO CORONAVIRUS/COVID-19
Lubbock Arts Alliance has put in place preventative measures to reduce the spread of COVID-19; however, Lubbock Arts Alliance cannot guarantee that you will not become infected with COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk I may become exposed to or infected with COVID-19 by participating in Lubbock Arts Alliance programming due to the actions and/or omissions of myself or others, including, but not limited, to Lubbock Arts Alliance volunteers, employees, Board members, sponsors, donor, patrons, and recipients of services.
I voluntary agree to assume all of the forgoing risks and accept sole responsibility to any illness to myself. I hereby release to hold harmless the Lubbock Arts Alliance of all liabilities of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on if a COVID-19 infection occurs before, during, or after any participation in any Lubbock Arts Alliance program.
